All swimmers/divers must have an AISD Physical Form signed by a physician on file before they can participate in any practices or meets.

The form must be signed by a physician AFTER April 15, 2012

- Austin Immediate Care (Slaughter Ln.) for $20.00
- Please make sure your swimmer/diver gets this taken care of before the start of the season
